WHAT COMPLEX NEEDS CAN WE SUPPORT?

We are experts in the specialist care of conditions that impact a person’s ability to live well.

This includes neurological conditions like Parkinson’s, multiple sclerosis or motor neurone disease, and physical disabilities such as muscular dystrophy and cerebral palsy. We also provide compassionate palliative care for those who are nearing the end of their life.

Our professional carers are expertly trained to provide nurse-led clinical interventions including respiratory and airway support, tracheostomy management, feeding support including PEG feeding and stoma and catheter care.

We work collaboratively with other healthcare professionals, multidisciplinary teams, commissioning groups and other therapy providers involved in our client’s care. This ensures an integrated and holistic approach underpins the adult complex care we provide.

Family involvement and support are central to our approach to providing adult complex home care. Our care teams ensure families receive high levels of support, as much as is needed. We take time to understand the caring relationship between our clients and their families and our approach is respectful of this and client-focused.

The level of care provided is planned in partnership with everyone involved in a client’s care as we know through our experience this ensures goals are reached and outcomes are improved.

WHAT IS NURSE-LED ADULT COMPLEX CARE?

Nurse-led care or complex care for adults is nursing care which is led and managed by an expert registered nurse who is highly experienced. The registered nurse assigned to a client will be responsible for developing and overseeing the clinical components of the care package. This ensures the carer who provides hands-on care has the requisite training, support and supervision to deliver high-quality home care safely and effectively.

As part of our nursing care service, we assess care needs and plan the care and support required whilst continually monitoring and evaluating outcomes. Our care is flexible and can be quickly adapted to meet changing needs.

Adult complex care includes:

  • Establishing and agreeing on clinical care needs
  • Undertaking risk assessments
  • Developing a plan of care, working with the client and family to set expectations
  • Providing hands-on care at home
  • Evaluating the care provided, ensuring the right care is delivered at the right time so a person’s needs are met – every time

Our adult complex care service is nurse-led and carers are trained to provide care and support for complex, terminal and other medical conditions that impact how you live your life.They are supported by an expert, dedicated registered nurse who provides unrivalled supervision and support for the clinical care we provide.

COST OF ADULT COMPLEX CARE

The cost of complex care for adults is dependent on the level of care and support you require. If you are receiving our hourly adult complex care the overall cost will be determined by the number of visits and frequency of visits to you at home. Our experts will conduct a comprehensive assessment of your needs at your home, or in hospital if you are awaiting discharge, to truly understand what you need and therefore how much complex care at home will cost.

We know that our cost of professional adult complex care is very competitive when compared with other home care providers.  It is worth noting that when comparing the cost of nursing care with a nursing care package in a nursing home, that the fee you pay for nursing care at home is actually for the care and support you receive.  In a nursing home your weekly fee mainly covers your accommodation, food and social activities with the care package being only a small part of what you pay.If you are living with a disability or life-limiting condition that affects your health and ability to live daily life, you may be entitled to either healthcare funding, social care funding or Government funding to contribute to the cost of complex care at home.  If you are faced with having to finance your own home care (known as self-funding care), there are initiatives available to you, including care fees annuities or equity release schemes that could provide the funds you need for your care and support

GIVING CHRIS HIS LIFE BACK WITH COMPLEX CARE AT HOME

Chris has been living with muscular dystrophy for many years. Recently his lung collapsed and following a tracheostomy, he required 24-hour care. Chris was determined to return home from hospital and live as independently as possible.

His critical medical condition gave him bouts of anxiety that impacted how he felt, so he needed to trust his care team. He worried about the loss of personal space from 24-hour adult complex care arrangement, so it was key that Chris and his carers got on well, so they could be honest with each other about the level of care he wanted.

Prestige Nursing & Care responded quickly with a team of carers to support Chris who would best meet his needs.  So that Chris could feel reassured and he had continuity we happily employed Chris’ live-in PA of 8 years, James, as part of the team. This approach enabled Chris to have control over his care team which was very important to him.  James was medically trained at the hospital, with additional training from Prestige.  Another carer was provided to deliver care in the morning and evenings, as well as a carer through the night to attend to his medical needs and personal care.

The complex care provided by the team at Prestige to support his muscular dystrophy symptoms included tracheostomy care, BPAP, suctioning, cough assist, ventilator and nebuliser support, handling and hoisting, as well as personal care and domestic care of his home.  Chris now lives a fulfilled life at home with his dedicated team of carers, and his mother is able to enjoy her life again.

Chris said: “My carers give me much-need emotional support, as well as practical help.  My views are completely respected and I feel understood.  My carers are more like friends, and most of all I have my life back.”

Our trusted and established care adult complex care service can be provided in the following ways for someone needing nurse-led care:

  • Hourly care or visiting care is for those who need care and support with tasks of daily living or clinical care at certain times during the day.  Care at home can be provided from a 30-minute visit to as many hours/visits throughout the day as you require. One day or several days of the week depending on your care and support needs. For those who have complex needs, hourly care can be provided over a 24-hour period.
  • Respite care for those needing a short-term arrangement after hospital discharge to rehabilitate or when a family carer needs a break from their caring role. It is also an ideal service for families who wish to assess whether care at home is the right care choice for them.

HELPING ROBERT’S SPINAL INURY REHABILITATION AT HOME

Robert sadly suffered a spinal cord injury having fallen down a flight of stairs, resulting in multiple fractures including a spinal fracture to his back.

Following the accident, he had surgery which involved the insertion of pins and plates as well as wrist fusions. This left Robert with paralysis from the chest down, with limited movement in his shoulders, his hands and wrists.
He also had Autonomic Dysreflexia directly related to his spinal cord injury, which causes high blood pressure coupled with slow heart beats. After the acute surgical phase, Robert was discharged from hospital to a specialist spinal injury unit for treatment and rehabilitation, but he was desperate to get back to his much-loved home.

Prestige Nursing & Care was contacted by Robert’s case worker with a view to establishing a 24-hour care and support package.  Robert required a team of fully trained carers to meet his complex needs around the clock.  They received bespoke training to meet his physical needs and additional clinical training to monitor and support his Autonomic Dysreflexia.  A plan of care was then developed for Robert that not only met his care needs, but reflected how he wished to live his life to support his independence, choices and wishes.

Prior to Robert moving home, the team at Prestige worked in partnership with the care management team at the spinal injury unit to ensure a smooth transition back home for Robert.  A clear action plan was created to ensure an efficient and effective discharge from the spinal injury unit, with the right adult complex care package in place.  Once home Robert’s carers completed training, with the expert oversight of Prestige’s registered nursing team to ensure they were competent to provide the care that Robert needed.  The local team worked with other healthcare professionals involved in Robert’s care to ensure he had essential equipment needed to improve the quality of his life at home.

After three years, Robert was able to move back home and his condition is well-managed by the Prestige care team, and its clinical experts. Living at home, with support, has enabled Robert to reclaim and retain as much independence as possible.

Complex Care FAQS

HOW DO I FIND OUT IF I AM ELIGIBLE FOR HEALTHCARE FUNDING?

If you are an adult living with a complex need and require nurse-led care, you may be entitled to healthcare funding to receive this care at home. NHS Continuing Healthcare funding is organised and funded by your local Clinical Commissioning Group (CCG).

The process of finding out whether you are eligible for NHS Continuing Healthcare funding can be a lengthy one as the qualification criteria is rigorous and can be overwhelming for some families.   As we have formed strong relationships with CCG teams locally, we are able to work effectively together to deliver the best outcome possible for our families.

So, what are the steps in securing healthcare funding:

  1. The first step is to ask for a continuing care assessment from your social worker or GP.
  2. A medical professional will be assigned to your application and will arrange with you to undertake an initial screening at home, or if you are in hospital, they will visit you there.
  3. Each of your needs if determined as healthcare needs, will be categorised as low, moderate, high, severe, or priority. You will be eligible for support if you have one priority and more than one severe need.

They will consider the following needs:

  • Breathing
  • Nutrition
  • Continence
  • Skin integrity
  • Mobility
  • Communication
  • Psychological needs
  • Emotional needs
  • Cognition
  • Behaviour
